New Patient Partnership Program At Grand Itasca

Grand Rapids, MN – Grand Itasca Clinic & Hospital is excited to announce a new patient partnership program called Include Always©. In partnership with Minnesota Hospital Association (MHA), Include Always©, offers an opportunity to transform health care by authentically engaging patients and families at all levels of their care.

 

Grand Rapids, MN – Grand Itasca Clinic & Hospital is excited to announce a new patient partnership program called Include Always©. In partnership with Minnesota Hospital Association (MHA), Include Always©, offers an opportunity to transform health care by authentically engaging patients and families at all levels of their care.

“We had already started a program like this internally when we were approached by MHA to partner on Include Always,” says Kayla Perkins, Patient Experience Coordinator at Grand Itasca. “Before we partnered with MHA, we were working on forming patient advisory groups, called Be Heard, to bring patients and staff together to discuss a variety of patient centered topics. When MHA asked us to partner, it was a natural fit for our organization.”

MHA is working with five Include Always pilot hospitals across the state: Glencoe Regional Health Services; Grand Itasca Clinic & Hospital; Sanford Bemidji Medical Center; Windom Area Hospital; and Lakewood Health System, Staples. The pilot hospitals are busy at work implementing patient engagement programs and will present lessons learned to all health care facilities statewide at the end of the pilot program.

“When the hospitals take that step, I have not heard one hospital say, ‘I wish we hadn’t brought those patients and families in.’ Every single hospital is so thankful that they took that step, and they are seeing benefits to it immediately,” said Lisa Juliar, Patient/Family Consultant at MHA.

Patients, families and front-line staff are invited to be a part of this innovative program to shape how Grand Itasca provides care. There are multiple ways to get involved through participating on a patient-centered committee or an advisory group, or allowing a Grand Itasca team member to shadow you throughout your clinic or hospital visit.
